Person-Centered Planning (PCP)

Your PCP is the foundation of the Self-Determination Program. I work closely with you to understand strengths, needs, goals, preferences, and long-term visions.

Together, we create a meaningful, strengths-based PCP that:

  • Reflects your child’s unique identity and abilities

  • Highlights what’s working and what’s not

  • Defines clear goals and needed supports

  • Ensures the plan aligns with your family’s culture and values

This plan becomes your guide for services, budget decisions, and accountability.

Independent Facilitation

Navigating the Self-Determination Program can feel overwhelming. I support you through each step to ensure the process is clear, manageable, and empowering.

Independent Facilitation services include:

  • Helping you understand the SDP process

  • Supporting completion or updates of the PCP

  • Assisting with Spending Plan changes

  • Identifying services that align with your goals

  • Helping you choose, hire, and manage providers

  • Advocating for needed supports during meetings

  • Ongoing guidance as your needs evolve

Advocacy Support

Whether you're navigating school, Regional Center, or community services, you deserve someone who understands the system and stands in your corner.

Advocacy support may include:

  • Preparing for IEP, IFSP, or school meetings

  • Identifying gaps in services or supports

  • Reviewing reports, goals, or recommendations

  • Helping you communicate needs clearly

  • Clarifying your rights

  • Ensuring your child’s needs and strengths are centered

My advocacy is grounded in lived experience as a support-needs mom and clinical knowledge as a pediatric OT. I help families step into meetings feeling prepared, supported, and empowered.
